At Community Care, we aim to create the best possible experience for older adults. We believe that providing quality care can enable older adults to live in their own communities, outside of institutions, even through the end of life.
Our approach is to seek solutions for individual participants’ needs. Our interdisciplinary teams of doctors, nurses, social workers, therapists and other health care workers put the best interests of those we serve at the forefront of decision-making. It drives us to be creative, innovative and cutting edge in practically everything we do. Team members get to know our participants well and use that knowledge to provide individualized, comprehensive, integrated and seamless care.
Our mission is to develop and demonstrate innovative, flexible, community-based approaches to care for at-risk populations, with an emphasis on the low-income population. Our goal is to better the quality of life for the populations we serve while optimizing the allocation of community resources.
Community Care began in 1977 when founder Kirby Shoaf created an organization to design, develop and operate a comprehensive, coordinated system of in-home, community-based services for the functionally disabled and the frail elderly. Since then, Community Care has established and operated more than a dozen programs. Each of these programs were designed to provide quality care and enhanced opportunity for quality of life to eligible seniors, while preserving their dignity and autonomy.
