The counties to be served by Community Care include: Sheboygan beginning operations on February 1, 2008, followed by Ozaukee county in March 2008, Washington county in April 2008, and Waukesha county in July 2008. Walworth county operations will begin in 2009. All counties will operate the Family Care program, others will operate the Wisconsin Partnership Program and/or the Program of All Inclusive Care for the Elderly (PACE). These programs serve elderly, physically disabled and developmentally disabled populations.
On January 1, 2007 Family Care services to residents of Racine county began. After one year of operation the wait list in Racine county no longer exists! As of February 1, 2007 Family Care services became available to the residents of Kenosha county. The Wisconsin Partnership program started serving Kenosha residents in April 2007.
It was a big day for Community Care on February 2 as the Governor presented a planning grant check at our Kenosha site. Leading the partnership named, Southeastern Wisconsin Care Managment Organization is Community Care Chief Operating Officer, Paul F. Soczynski. He along with representatives of Kenosha, Racine, Washington, Waukesha, Ozaukee counties and 3 private organizations will partner together to expand long-term care services for the elderly, physically and developmentally disabled. "The planning grant will support our mission of Community Care, which is to keep people in their homes while making wise use of the limited funds available" says Paul. Many staff and two of our Racine participants were present at the event.
After searching for a very long time a building that was formerly the home to the Industries for the Blind was purchased by Community Care on June 29. The building had been used for light manufacturing and offices. The 95,000 square foot building will be renovated to allow for many uses including: an adult day health center, a clinic, general office space, a kitchen, central storage and employee fitness center. Torke, Wirth and Pujara is the architect working on the project. JENS Construction is the General Contractor. The facility was completed in May 2007.
