We thrive on the synergy of collaboration
One of the keys of our strategic planning is an emphasis on sharing our existing infrastructure, while at the same time seeking to continuously improve our model. We know how to make a difference in the lives of older adults and appreciate the synergy that results from collaborating with like-minded organizations. By collaborating with agencies, we can serve more people. By reaching out to help others, we also energize ourselves to do what we do better.
Our partnerships:
- The Lapham Park Venture
Community Care joined with the City of Milwaukee Housing Authority, Milwaukee County Department on Aging, S.E.T. Ministry, Columbia St. Mary’s Family Practice Clinic and the Lapham Park Resident Organization to create a pioneering continuum of care facility. Through the Lapham Park venture, they created a model Continuing Care Retirement Community in a publicly funded housing development, giving a high quality alternative to nursing home placement for economically disadvantaged elderly.
- Connecting Caring Communities
Community Care teamed up in 2004 with the Milwaukee County Department on Aging, Interfaith Older Adult Programs, the Milwaukee Aging Consortium, the Wisconsin Geriatric Education Center and Marquette University to make life more “elder friendly” in Milwaukee County. The multifaceted project aims to help older adults who need long-term care because of physical or cognitive impairments, as well as those 60 and older who are at increased risk of disability because of poverty, chronic illness or advanced age. This innovative collaboration won $1.4 million in grants from the Robert Wood Johnson Foundation.
