Who
is a good candidate for Community Care’s PACE and Partnership
programs?
- They have had several trips to the hospital in the past year.
- They have trouble managing their medications.
- They have trouble managing their health care (making doctors
appointments, arranging transportation, follow up after doctors
appointments).
- They need help with activities like driving, shopping, making
meals, and they may also need help with activities like bathing
and dressing.
- They are calling their doctor’s office often.
- They need closer medical monitoring by primary care.
Because the PACE and Partnership teams are able to provide hands
on primary and nursing care, they are often able to help stabilize
a clients medical conditions.
