Community Care prepares to launch services in Sheboygan County
Community Care will begin 2008 by launching Family Care in Sheboygan County.

Community Care has been chosen by the state to expand Medicaid-funded, managed long-term care services in Ozaukee, Sheboygan, Washington and Waukesha counties in 2008, and Walworth County in 2009. This expansion of services is scheduled to roll out over the year, beginning with the launch of Family Care in Sheboygan in February.

Family Care serves adults with physical or developmental disabilities, as well as the elderly. It serves Medicaid-eligible adults who have long-term care needs expected to last more than 90 days and are at least 65 years old, or are 18 years old or older with a physical or developmental disability.

Family Care members receive support from a Community Care team that includes a nurse and a case manager. Each Community Care team conducts a comprehensive assessment of the member’s needs, abilities, preferences and values to coordinate the right long-term care services for that individual. Family Care members continue choosing their own physicians.

Family Care offers a wide variety of non-medical long-term care services, including:

  • Personal care
  • Homemaking and chore services
  • Occupational and physical therapy except during an inpatient stay
  • Home-delivered meals
  • Specialized equipment
  • All Medicaid-funded, long-term care services

A look ahead at a year of expansion
Following the February launch of Family Care in Sheboygan, Community Care is planning to expand programs into four more counties in southeastern Wisconsin: Ozaukee, Walworth, Washington and Waukesha.

In addition to Family Care, Community Care will also make the Family Care Partnership Program available.

Family Care Partnership from Community Care provides a full range of long-term care, primary, acute and preventive care delivered through interdisciplinary teams. A Community Care team, which includes a registered nurse, licensed social worker, nurse practitioner and doctor, ensure a participant’s complete social, personal and medical care needs are met.

Plans are in the works to roll out these programs as follows:

  • February: Family Care in Sheboygan County
  • March: Family Care in Ozaukee County
  • April: Family Care in Washington County and Family Care Partnership in Ozaukee County
  • May: Family Care Partnership in Washington County
  • July: Family Care in Waukesha and Family Care Partnership in Waukesha County
  • January 2009: Family Care Partnership in Sheboygan County and Family Care in Walworth County
