Charles Hajinian is a participant in the Racine Partnership
program from Community Care. Charles reports he is happy with the
care he receives from Community Care.
“They are very concerned about each individual,
about how you’re doing both physically and mentally,” says
Charles. “If I need anything, I can call. They are always there
for you. Community Care is an organization that has a lot of love
and consideration for each and every individual it serves.”
Charles
still lives in the home where he lived for many years with his wife.
He is a self-sufficient, strong person who had always taken care
of everything, say those who know him. You’d find him painting
his home, tarring the roof, whatever maintenance was needed. He is
a veteran who proudly served his country.
The Racine Partnership program helps him remain
as independent as possible. He receives assistance with medical and
medication management and other services as needed.
The Racine Partnership
program from Community Care provides a full range of long-term care,
primary, acute and preventive care delivered through interdisciplinary
teams. Each team includes a registered nurse, licensed social worker,
nurse practitioner
and doctor who work together to ensure members’ complete social,
personal and medical care needs are met.
