New webpage created just for providers
Community Care just added a new section to our website — a page just for providers. This new page contains links to key documents and resources for providers, including directories of current Community Care providers. And now you can easily access Community Care forms for billing and deposits and Form W-9 through the website. Those interested in becoming Community Care providers can also submit an inquiry form online.
