Community Care launched Family Care in Sheboygan, Ozaukee and Washington counties
in early 2008, expanding Medicaid-funded, managed long-term care services for
frail older adults and adults with disabilities.
The State of Wisconsin contracted
Community Care to develop long-term care services to meet what has
long been a shortfall of services. For frail older adults and adults
with disabilities in these three counties, it means the end of waiting
lists for long-term care services are in sight.
Community Care launched
Family Care in Sheboygan County in February, in Ozaukee County in March
and in Washington County in April.
Community Care is partnering with
Sheboygan, Ozaukee and Washington counties to provide Family Care,
which will offer long-term care services that allow eligible seniors
and adults with disabilities to live where they prefer: in their homes
and communities.
Family Care members are receiving support from a Community
Care team made up of a nurse and a case manager who conduct a comprehensive
assessment of the member’s needs, abilities, preferences and
values to coordinate the right mix of long-term care services for that
individual.
With care provided by interdisciplinary teams that include
medical professionals, Family Care targets prevention and planning
in addition to treatment. The program also offers a wide variety of
non-medical, long-term care services to members, such as homemaking
and chore services and home-delivered meals.
