Community Care’s PACE Program offers what you need when you need it
PACE or Program of All-inclusive Care for the Elderly designed to meet the long-term health care needs of frail older adults and the disabled. In these programs, ongoing medical care, social and therapeutic services are provided to participants through Community Care’s Adult Day Health Centers in coordination with contracted specialty provider and in-home care. Community Care provides a community-based continuum of care to enable persons whose physical and/or cognitive status make them eligible for nursing home placement to remain at home for as long as is practical.

Community Care’s Wisconsin Partnership Program keeps you in the community
With our Wisconsin Partnership Program, many services come to you and you can visit a doctor in your community.

Both PACE and Community Care’s Wisconsin Partnership Programs offer a variety of program benefits that are given according to assessed need. They include:

  • Homemaker Services
    Community Care homemakers or chore service providers will help you with chores at home.
  • Home Health Care
    Community Care’s home health care services include a variety of medical and non-medical services to help participants remain independent in their homes. These services could range from highly skilled care provided by nurses to aides who provide non-medical assistance, such as help with personal care like bathing and dressing.
  • Pharmacy
    Community Care offers pharmacy services for the convenience of its participants. PACE participants are able to receive their prescriptions, as well as other over-the-counter medications, through the Adult Day Health Center. Partnership Program participants may have medications delivered to them at home or through the Adult Day Health Center, should they choose to attend.
  • Adult Day Services
    When an older person is not homebound but still requires some guidance and supervision, adult days services could be an ideal solution. For a few hours each day, participants may go to a Community Care Adult Day Health Center where a variety of services ranging from clinic visits and care to programs designed to encourage socializing and interaction with others are available.
  • Rehabilitation Therapy
    Community Care locations offer a variety of therapy options for participants needing a range of physical and occupational rehabilitation and speech therapy.
  • Behavioral Health Care
    An interdisciplinary team that includes a psychiatrist, a social worker and a psychiatric nurse provide behavioral health as well as substance abuse services.
  • Social Services
    Community Care provides support and problem solving to participants and families adjusting to the aging process. We connect them with a variety of resources both through Community Care and in the community.
  • Transportation
    Community Care’s fleet of vans is available to participants for medical appointments or Adult Day Health Center visits. Our drivers are trained to provide familiar, yet safe access for participants. We will also assist in arranging transportation for some social visits.
  • Home Meal Delivery
    For participants needing help with preparing meals or having meals delivered, Community Care is there to help by providing meals at home. Meals are also served in our Adult Day Health Centers.
  • Respite for Caregivers
    Many of our participants are able to remain independent because of the tremendous help and support they receive from a family member of friend. Because care giving can sometimes be a physically and emotionally demanding responsibility, Community Care offers seven days of respite services a year to provide a much-needed break to these informal caregivers.
  • Dental Care
    Community Care has an onsite dentist who offers participants convenient access to preventive and medically necessary dental care, including the treatment of and replacement of natural teeth due to normal activities of daily living.
  • Vision Care
    Participants access to vision care, including optional routine optical care, at all Community Care sites.
  • Hearing Care
    Hearing exams, routine hearing screenings and hearing aids are available to participants at all Community Care sites.
  • Foot Care
    Routine foot care services are available to participants at all Community Care sites.
  • Pastoral Care
    Nondenominational pastoral care services are also available at all our sites to those who request it.
  • End of Life and Palliative Care
    Community Care’s end-of-life and palliative care services help terminally ill people remain as comfortable and vital as possible in a familiar home setting while providing relief from pain and discomfort.
