Community Care offers two programs, PACE or Program of All-inclusive Care for the Elderly and the Wisconsin Partnership Program, as innovative alternatives to nursing home care for eligible participants.

To be eligible for the PACE Program, you must:

Be at least 55 years old
Live in Milwaukee County
Meet a nursing home level of care
Be eligible for Medicaid and/or Medicare

To be eligible for the Wisconsin Partnership Program, you must:

Be at least 55 years old
Live in Milwaukee or Racine County
Meet a nursing home level of care
Be eligible for Medicaid or for Medicaid and Medicare benefits

Those eligible for Medicare must have both parts A and B, and continue to pay their part B premium.

Enrollment means security

Once enrolled in Community Care, you remain enrolled for life, regardless of your changing health needs. As a Community Care participant, your needs are met, whether you need hospitalization or nursing home care.
