Q: Do I have to change my physician?
A: You may need to change your physician if he or she is not in our network. Community Care maintains a network of quality physicians including onsite primary care doctors and contracted specialty care physicians. If your physician is in our network, you will be able to keep your physician if you choose.
Q: How much is this program going to cost me?
A: Community Care receives monthly per capita (or per person) payments from Medicaid, and where applicable, from Medicare, for all eligible enrollees. Most PACE participants who qualify for both Medicaid and Medicare, pay no out-of-pocket costs for services. Wisconsin Partnership Program participants make a small copayment for prescription drug medications, and there are no other out-of-pocket costs for services. Once enrolled in PACE or the Wisconsin Partnership Program, other than the possible monthly cost share, you will not be chaarged for any of your health care services.
Before enrolling, you will have the opportunity to discuss your financial situation with our enrollment specialist and you’ll know exactly what your costs will be.
Q: How long does it take to be enrolled?
A: About four to six weeks.
Q: What is the difference between PACE and Wisconsin Partnership?
A: In the PACE program, you see your physician in our Adult Day Health Center. In the Partnership program, you see a physician located in your community.
Q: How can I get more information about how Community Care programs will meet my needs?
A: You may call (414) 385-6600 to discuss your questions or make an appointment to have an enrollment specialist visit you in your home. For toll-free or TTY access and hours of operation, click here.
