Community Care’s unique programs help you live life on your terms
For Ann K., enrolling in a Community Care program was “a Godsend.”

A quadriplegic due to multiple sclerosis, Ann says, “If I wasn’t here and didn’t have the family support I do, I would be in a nursing home.” Community Care’s programs and support from her husband Tom have enabled Ann to enjoy the best of health care, to live at home, to take on important roles in her community and to spend time with people she enjoys.

Community Care provides two comprehensive programs that offer alternatives to nursing home care: PACE (Program of All-inclusive Care for the Elderly) and Community Care’s Wisconsin Partnership Program. PACE serves people who reside in Milwaukee County and Wisconsin Partnership Program serves Milwaukee and Racine counties. In the PACE program, participants see a Community Care staff doctor; in Partnership, participants have the opportunity to remain with their community-based physician, if he or she is on our list of health care providers.

“It is all encompassing, excellent care,” says Ann, who serves on the Community Care Board of Directors. “If someone in these programs has an ingrown toenail or a lump in the breast, they can be seen by a doctor right away. We have ready access to our doctors and nurses. Otherwise, it could take days, weeks even, to get it evaluated by a doctor.”

Community Care’s PACE Program offers comprehensive package of services coordinated through an interdisciplinary team. Participants receive all the services they would receive in nursing homes while remaining in their own homes. Many of the services are provide in our Adult Day Health Centers. One of the goals of these programs is to provide aggressive preventive care that maintains our participants’ health and avoids high-cost institutional care.

The Wisconsin Partnership Program also offers comprehensive health services to older adults. However, it allows participants to select a community-based physician through our network of providers, keeping their own doctors if he or she is part of the network.

At Community Care, each participant has a healthcare team

When you enroll in Community Care, you will get your own interdisciplinary team to serve you. Your interdisciplinary team will design an individualized care plan for you and will continue to see you regularly and monitor your health. What makes this approach work so well is the consistent communication your team will have with you and each other about your health care needs.
