Community Care’s staff will walk you through the simple enrollment process. You may start the process by simply calling (414) 385-6600 in Milwaukee and (262) 633-4800 in Racine. A Community Care enrollment specialist will talk with you to gather basic information about your health care needs and finances in order to determine if you meet the eligibility requirements.
Each caller who wishes to pursue enrollment will be assigned to a particular enrollment specialist. That staff member will contact you to collect more information and to make an appointment for a home visit.
During that home visit, a detailed screening of your health needs, safety and personal concerns, physical and cognitive abilities and financial status is completed. This allows Enrollment staff to complete a computer-based Functional Eligibility Screen that informs them whether you are functionally eligible for our program. During that visit, you will also be asked to give us permission to verify health information through a review of your medical records and begin an initial review of your Medicaid eligibility.
Information is then shared with members of our Financial Eligibility Department, whose job it is to work with the County to determine eligibility for Medicaid.
Next you will be scheduled for an assessment with an interdisciplinary team, the very same team that will care for you when you are in the program. Think of it as test driving the program to see how you like it.
- If you’re enrolling in PACE or Program of All-inclusive care for the Elderly, you will come into the Community Care Adult Day Health Center and meet with a physician, registered nurse, social worker, rehabilitation therapist, activity therapist and other members of the team.
- If you are enrolling in our Wisconsin Partnership Program, a nurse practitioner, a registered nurse and a social worker will visit you in your home to assess your health care needs.
The team will meet and develop a care plan especially for you. You will have the opportunity to review it and decide if you’d like to enroll. The whole process usually takes from four to six weeks.
Throughout this process, you will be guided and supported by the experience and compassionate people in Community Care’s Enrollment and Financial Eligibility Departments.
