PACE participants at this site have all their health care needs met under one roof. In addition, there are social activities, physical therapy, home health services, daily meals and much more.

Separate PACE and Wisconsin Partnership Program teams provide services to participants in both programs. The PACE team works with onsite physicians, while the partnership team partners with physicians at Columbia — St. Mary’s Family Practice Clinic, the Aurora Family Care Center and the Geriatric Institute. These teams serve more than 200 people, mostly in the community, while providing integrated primary, acute, specialty and long-term care.
