At Community Care, we believe caring for frail elders and adults with disabilities in their communities. It is good for the people we serve and good for the community. That’s why we have chosen to expand our services in Southeastern Wisconsin to provide care for frail elders and adults with disabilities.

Our unique approach to caring for the frail elderly and disabled provides an innovative alternative to institutional care. It provides a way for those who require long-term care to remain living in their own homes for as long as possible while receiving all the services they need.
