| Today our Layton Boulevard site has two PACE teams providing services to participants. These teams provide integrated primary, acute, specialty and long-term care. PACE participants visit the Adult Day Health Center, located in what was the Sacred Heart Rehabilitation Hospital on 27th street, just south of the famous Milwaukee “Domes.” It is part of a complex owned and operated by the School Sisters of St. Francis, who live within the same building. A partnership has grown between Community Care and the sisters. Community Care and the School Sisters share a sense of mission about serving this community, which has a growing Hispanic population. Consequently, we celebrate many Hispanic holidays and have a number of bilingual staff members. This sharing of cultures enriches the lives of those served at this center.
Layton Boulevard also has a Wisconsin Partnership Program team that provides services to participants by partnering with physicians at the Geriatrics Institute, Aurora Family care Center, St. Luke’s Family Practice and Milwaukee Family Practice.
The building itself has quite a history. It was once used as a "spa/clinic" for "well-to-do" clients. Then, as the home to the School Sister of St. Francis, it has unique character. The sisters enjoy art, so the hallways and room walls are filled with paintings and sculptures created by sisters themselves. It is a joyful place to work and a wonderful environment for the clients that attend the Adult Day Health Center.
