| Community Care on Prospect Avenue, located on the east side of Milwaukee, used to be a traditional style nursing home. This four-story building is now totally remodeled as a Community-Based Residential Facility (CBRF), removing the old institutional look it had as a nursing home.
It is used as a group home, with specialized living areas for dementia clients and a group home for members that are too medically frail to live on their own. Social activities, physical therapy, home health services, daily meals and much more are all found at this location. It features the “Age in Place” model, allowing existing Community Care participants to progress to this setting, depending on their needs.
Community Care on Prospect Avenue is only open to PACE and Wisconsin Partnership Program participants from other Community Care locations.
In 1995, Community Care purchased and opened this group home in order to provide for participants who were no longer cognitively or medically able to live in the community independently. In 2002, we added a Wisconsin Partnership Program team to the Prospect site to serve those participants who wished to remain with a contracted community physician.
