| Community Care helps Racine seniors live at home |

| The Racine Partnership program — part of the Wisconsin Partnership Program — targets low-income, nursing home-eligible older adults living in the community, offering treatment options to allow them the opportunity to remain independent. Community Care provides case management in collaboration with the following service providers in Racine County: All Saints Health Services, Lincoln Lutheran Home, Aurora Health Care and Society’s Assets. The full range of Wisconsin Partnership services, including home care, rehabilitative therapies and adult day care are provided to medically frail seniors in Racine County.
