Community Care collaborated with the Milwaukee County Department on Aging, S.E.T. Ministry, Columbia — St. Mary’s, and the Housing Authority to create an urban retirement community. The Lapham Park site serves Wisconsin Partnership participants who reside in the Lapham Park HUD building. The Lapham Park Venture has led to several awards for modeling a program of community-based health care within public housing.

The Lapham Venture was awarded a grant, which led to renovating the lower level of the building to resemble the Walnut Street of years ago, where many of the current residents spent their youth. The lower level retirement center not only houses our Partnership Clinic, but also serves as a meal site and a recreation center. We are very proud of this collaboration and the manner in which it has enhanced our Partnership program.
