Summary: Provides case management services including intake and assessment, making referrals, service plan development and ongoing monitoring of services to clients in the Manitowoc County Long Term Support Program.

Qualifications: This position requires a Bachelor’s degree in Nursing, Social Work, Sociology, Vocational Rehabilitation, Psychology or related human service field and one year experience with older adults in a social service capacity.
