Summary: Provides primary health care to participants of Community Care Programs, including assessment and development of plan of care in collaboration with the interdisciplinary team, provision of direct patient care and evaluation of effectiveness of plan of care. Functions in a collegial and collaborative relationship with physicians and other health professionals making independent decisions about nursing needs and collaborative decisions with physicians regarding health regimens. Assumes dependent responsibilities in carrying out delegated medical acts. Qualifications: This position requires a master’s degree in nursing with completion of an accredited program for nurse practitioners. ANCC certification (GNP, ANP, FNP), Wisconsin Advanced Practice Nurse Prescriber license, DEA registration (or eligibility), and at least two years of clinical nursing experience. A minimum total of one year of personal, clinical/internship, and or employment experience providing care or services for a frail or elderly population also required. A valid WI drivers license and use of a personal vehicle are also necessary for travel between Community Care sites, and to contracted nursing homes and hospitals as well as participant homes. |
