Summary: Provides personal care and services necessary for attending to the personal needs, support and health status of Community Care program participants.

Qualifications: This position requires State of Wisconsin nurse aide certification and a minimum total of one year of personal, volunteer or employment experience providing care or services for a frail or elderly population required.  Experience working with individuals with dementia is a plus.
