Summary: Provides personal care and homemaking services for assigned Community Care program participants primarily in the home environment.

Qualifications: This position requires State of Wisconsin nurse aide or home health aide certification and a minimum total of one year of personal, volunteer or employment experience providing care or services for a frail or elderly population required.  Experience working with individuals with dementia a plus.
