Physician

Summary: Provides primary medical care to participants in the Community Care Programs, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct patient medical care and evaluation of effectiveness of the plan of care.  The Physician functions in a collegial relationship with the other health professionals, making independent decisions regarding medical needs and health regimens, and carries out medical procedures as indicated.  The quality of the medical care is under the direction of the CMO and Associate Medical Directors.

Qualifications: This position requires licensure to practice medicine in the State of Wisconsin, DEA registration, ability to obtain and maintain staff privileges at Community Care contracted hospitals, and a minimum of one year primary care experience with a frail or elderly population. Board Certified in Internal Medicine or Family Practice; advanced certification in geriatrics preferred.
