Summary: Functions as a member of the Community Care’s Care Management Inter-disciplinary Team. Develops client service plans and performs on-going coordination, and monitoring of services for clients enrolled in the Family Care Program. Assists other team members in identifying and managing clinical issues of clients during assessment, and service plan development and implementation. Routine travel between Community Care sites and in the community is required.

Qualifications: This position requires a degree/diploma (BSN preferred) in nursing, WI RN license, and two years recent experience with older adults in a hospital, homecare setting or nursing home experience.  Two years of Community nursing preferred. A valid drivers license is required and use of a personal vehicle is necessary.
