Summary: Functions as a member of Community Care’s interdisciplinary team in planning, coordinating, and providing health care services for program participants. Primary responsibilities include assessing, coordinating, monitoring, and providing clinical treatment and services for Community Care’s Program of All-Inclusive Care for the Elderly (PACE) and/or Partnership Program participants. Facilitates the integration of new participants into the Community Care health care delivery system and assists in development of participant plans of care. Occational to routine home visits are required (frequency determined by assigned PACE or Partnership Program caseload). Qualifications: This position requires a degree or diploma in nursing (BSN preferred), WI RN license, and two years experience in clinical nursing with a frail or elderly population. Experience in a community-based or long-term care setting is preferred. A valid drivers license is required and occasional to routine use of a personal vehicle is necessary. |
