RN Functional Eligibility Screener

Summary: Responsible for completing all annual and change-of-condition functional screens on participants of Community programs for an assigned region according to established Community Care Program of All-Inclusive Care for the Elderly (PACE) and Partnership program guidelines.

Qualifications: A registered nursing degree, current State of Wisconsin Registered Nurse license, and a minimum of three years recent experience in clinical nursing with a frail or elderly population required. Department of Health and Family Services (DHFS) Functional Eligibly Screener certification required within one month of hire.   Experience in a community-based or long term care setting involving the elderly preferred.
