Summary:

Functions as a member of Community Care’s interdisciplinary team in planning, coordinating, and providing health care services for program participants.  Primary responsibility is to provide direct social work case management services for Community Care’s Program of All-Inclusive Care for the Elderly (PACE) and/or Partnership Program participants. Occasional to routine home visits are required (frequency determined by assigned PACE or Partnership Program caseload).

Qualifications: This position requires a Bachelor’s degree in social work and current Wisconsin Social Worker certification.  One year of experience providing care or service for the elderly and a Master's Degree in Social Work are preferred. A valid drivers license is required and occasional to routine use of a personal vehicle is necessary. Must be bilingual in Spanish/English.
