Summary: Functions as a member of Community Care’s interdisciplinary team in planning, coordinating, and providing health care services for program participants. Primary responsibility is to provide direct social work case management services for Community Care’s Program of All-Inclusive Care for the Elderly (PACE) and/or Partnership Program participants. Occasional to routine home visits are required (frequency determined by assigned PACE or Partnership Program caseload).

Qualifications: This position requires current Wisconsin Social Worker certification and a Master’s degree in social work and one to two years experience providing social work case management services to a geriatric population. A valid drivers license is required and occasional to routine use of a personal vehicle is necessary.
