Functions as a member of Community Care’s interdisciplinary team in planning, coordinating, and providing health care services for program participants. Primary responsibility is to provide physical therapy services for participants of Community Care’s Program of All-Inclusive Care for the Elderly (PACE) and Partnership Program. Performs in initial and annual screenings/assessments and develops and implements treatment plans. Directs the clinical work of the PTA(s). Routine travel between Community Care sites and in the community is required.
This position requires a Bachelor's/Master’s degree in PT, a current Wisconsin PT license, and one year experience working with a frail elderly population (appropriate clinical placement experience may be substituted). A valid drivers license is required and use of a personal vehicle is necessary.
