The Family Care program from
Community Care helps frail seniors and adults with physical or
developmental disabilities live at home, among friends and family.
Our
Family Care members receive services where they live. Each of
our Family Care members has support from a Community Care team
made up of a nurse and a social worker. Each member’s team
conducts a comprehensive assessment of the member’s needs,
abilities, preferences and values. The members themselves remain
responsible for their own medical services, including choosing
their own physicians, while Family Care provides the additional
long-term care services they need.
Family Care offers a wide variety of long-term
care services, making it possible for seniors and adults with physical
and developmental disabilities to live at home and stay connected
to their community.
These services include:
- Personal
care
- Homemaking and chore services
- Occupational
and physical therapy (except during inpatient stay)
- Home
delivered meals
- Specialized equipment
- Other Medicaid-funded
services, as needed
