The hallmark of Community Care’s approach to care is that it is totally integrated. Unlike traditional health care and social services, Community Care’s approach integrates long-term, primary, acute and preventive care through the use of interdisciplinary teams. A single team sees participants through assessments and care delivery and coordination, and the result is that participants enjoy comprehensive, continuous, total care that addresses their changing needs.

Community Care offers this approach to care through two programs:

1. PACE or Programs of All-inclusive Care for the Elderly
2. The Wisconsin Partnership Program

Community Care also operates as a Family Care - Care Management Organization in Racine, Kenosha, Sheboygan, Ozaukee, and Washington Counties
