Community Care Partners offers a full range of services related to marketing, financial, operational and legislative analysis for organizations interested in developing integrated care models, such as PACE, or organizations interested in innovatively expanding current delivery systems.

Community Care has a long history of providing PACE Technical Assistance Center. We provide these services because we want to encourage replication of successful approaches to long-term care.
